The following information is for LegalShield members only. To begin your will questionnaire, download the form, and fill it out completely. You can then email the completed form to indocs@okoonlaw.com for Indiana members, or kydocs@okoonlaw.com for Kentucky members. In the subject line, please write your full name and “Will Questionnaire” (ex. John Doe Will Questionnaire) and attach the form. This will questionnaire is NOT your will; it is designed to help us create your will. After we receive your form, our firm will contact you regarding the next steps.
